I have frequently seen the statement that stroke is the number one source of disability in the US.
A search of the WEB does not support that statement.  
Stroke appears to be a primary, but not #1 source of disability

What are the most common types of disability in the U.S.?
From 1992 National Health Interview Survey, 
    1)     Heart disease, ( 7.9 million)= 13% of all disability reported. 
    2)     Back problems rank second (7.7 million), 
    3)     Arthritis (5.7 million), 
    4)     Orthopedic impairments of lower extremity (2.8 million)
    5&6) Asthma and diabetes (2.6 million each). 
    7)     Mental disorders rank (2.0 million), 
    8)     Disorders of the eye (not including visual impairments)
    9/10) Learning disabilities/mental retardation (1.6 million each). 
    11/12) Cancer and visual impairments each limit 1.3 million Americans
    13/14) Orthopedic impairments of the shoulder and/or upper extremity limit 1.2 million. 
    15)    Hearing impairments affect 1.2 million in the U.S.

Ischaemic heart disease goes from 4th cause of disability in 1990 to first in 2020
     It is the top cause of death in both 1990 and 2020              added 09/23/00

arthritis or rheumatism (mentioned by 7.2 million persons)
back or spine problems (5.7 million)
heart trouble (4.6 million)
lung or respiratory trouble (2.8 million)
high blood pressure (2.2 million)
stiffness or deformity of the foot, leg, arm, or hand (2.0 million)
diabetes (1.6 million)
blindness or vision problems (1.5 million)
